Note: People with HIV infection are advised not to breastfeed. Formula feeding is the safest choice to minimise HIV transmission; however, issues such as affordable and safe access to formula should also be considered and an approach based on individualised counselling, shared decision-making and risk-reduction should be adopted. In countries in which no acceptable, feasible, sustainable and safe replacement feeding is available, exclusive breastfeeding for 6 months is recommended to reduce the risk of HIV transmission from the breastfeeding person to the infant. Mixed feeding (combination of breastfeeding and replacement [formula] feeding) is contraindicated at any time as it is associated with increased risk of HIV transmission from the breastfeeding person to the infant.
