Treatment of trichomoniasis

To treat trichomoniasis, useOng, 2023:

1metronidazole 400 mg orally, 12-hourly for 7 days metronidazole metronidazole metronidazole

OR if adherence to a 7-day regimen is unlikely

2metronidazole 2 g orally, as a single dose. metronidazole metronidazole metronidazole

Single-dose treatment for trichomoniasis is less effective and should only be used if adherence to the preferred 7-day regimen is unlikelyKissinger, 2018.

Symptomatic trichomoniasis in pregnancy should be treated using one of the regimens aboveWorkowski, 2021. There is no proven benefit of treating asymptomatic infection in pregnancy and routine screening is not recommendedWorkowski, 2021.

Metronidazole resistance in Trichomonas vaginalis can occur; if symptoms persist despite treatment, seek expert advice.

Advise patients toOng, 2023:

  • abstain from sexual contact for 7 days after treatment starts, or until treatment is completed and symptoms have resolved (if treating a syndrome or symptomatic infection), whichever occurs later
  • notify sexual contacts within the past 2 weeks of the need to seek testing.
