Intermittent vancomycin dosing in young infants

For young infants aged 0 to 90 days without abnormal kidney function, vancomycin pharmacokinetics are affected by postmenstrual age1, weight and serum creatinineGwee, 2019. A calculator incorporating these factors should be used to determine the intermittent vancomycin dose; use:

vancomycin intravenously; see the Vancomycin Intermittent Dosing Calculator for Young Infants2 for dose and frequency. vancomycin

For young infants with abnormal kidney function, a weight-based vancomycin dose is recommended. Give a single dose and seek expert advice for subsequent dosing; use:

vancomycin 15 mg/kg (actual body weight) intravenously, as a single dose. Seek expert advice for timing of the next dose. vancomycin

1 Postmenstrual age = gestational age (time between the first day of the last menstrual period and birth) + postnatal age (time since birth).Return
