Birthing parent has never taken nevirapine or has been taking nevirapine for 3 days or less
For neonates at high risk of perinatal transmission, if the birthing parent (eg mother) has never taken nevirapine or has been taking nevirapine for 3 days or less, use as a 3-drug regimen:
lamivudine 2 mg/kg orally, 12-hourly for 4 weeks lamivudine
PLUS
nevirapine 2 mg/kg orally, daily for 1 week, then 4 mg/kg daily for 1 week nevirapine
PLUS
zidovudine orally zidovudine
neonate born at 34 weeks gestation or more: 4 mg/kg 12-hourly for 4 weeks
neonate born at 30 to less than 34 weeks gestation: 2 mg/kg 12-hourly for 2 weeks, then 2 mg/kg, 8-hourly for an additional 2 weeks
neonate born at less than 30 weeks gestation: 2 mg/kg, 12-hourly for 4 weeks.
For neonates at high risk of perinatal transmission who do not tolerate oral medication, seek expert advice.
