Postoperative considerations

Strict maintenance of closed-catheter drainage and early catheter removal reduces the incidence of urinary tract infection in patients with a temporary indwelling catheter. Do not extend the duration of antibiotic prophylaxis because the patient has a urinary catheter in situ—this practice is not supported by evidence and may cause adverse effects, including subsequent infection with resistant pathogens and Clostridioides difficile.

At the time of writing, there are inadequate data to recommend postoperative oral antibiotic prophylaxis to prevent pyelonephritis after cystectomy with ileal conduit or neobladder.
