Dosage of fondaparinux

Fondaparinux is a synthetic, antithrombin III–mediated, selective inhibitor of factor Xa. Fondaparinux is predominantly excreted via the kidneys so if it is required for a patient with a calculated creatinine clearance (CrCl) less than 50 mL/min seek advice from a specialist. Fondaparinux should not be used in patients with a CrCl less than 30 mL/min.

Fondaparinux is given subcutaneously once daily, but the dosage varies according to the indication. For example:

• heparin-induced thrombocytopenia (HIT)
• venous thromboembolism prophylaxis, including for patients who have had prior HIT
• venous thromboembolism treatment in patients with known or suspected HIT
• acute coronary syndromes; see the fondaparinux product information for dosage
• superficial vein thrombosis.
