Dosage of fondaparinux

Fondaparinux is a synthetic, antithrombin III–mediated, selective inhibitor of factor Xa. Fondaparinux is predominantly excreted via the kidneys so if it is required for a patient with a calculated creatinine clearance (CrCl) less than 50 mL/min seek advice from a specialist. Fondaparinux should not be used in patients with a CrCl less than 30 mL/min.
