Dosage of low molecular weight heparin
The route of administration and the dosage of low molecular weight heparin (eg enoxaparin, dalteparin) varies according to the indication. For example:

- patients with atrial fibrillation who are to undergo electrical cardioversion or pharmacological cardioversion
- venous thromboembolism prophylaxis
- venous thromboembolism treatment (see Low molecular weight heparin for VTE treatment and Thrombolytic therapy and interventional procedures for pulmonary embolism)
- superficial vein thrombosis
- periprocedural bridging therapy of warfarin
- anticoagulation during diagnostic and therapeutic cardiovascular procedures.
