Dosage of unfractionated heparin

The route of administration and the dosage of UFH vary according to the indication. For example:

• acute coronary syndromes (see Parenteral anticoagulation for thrombolytic therapy for STEMI, Parenteral anticoagulant therapy for NSTEACS and Mural thrombus following myocardial infarction)
• patients with atrial fibrillation who are to undergo electrical cardioversion or pharmacological cardioversion
• venous thromboembolism prophylaxis
• venous thromboembolism treatment (see Unfractionated heparin for VTE treatment and Thrombolytic therapy and interventional procedures for pulmonary embolism)
• periprocedural bridging therapy of warfarin
• anticoagulation during diagnostic and therapeutic cardiovascular procedures.
