Dosage of unfractionated heparin
The route of administration and the dosage of UFH vary according to the indication. For example:
- acute coronary syndromes (see Parenteral anticoagulation for thrombolytic therapy for STEMI, Parenteral anticoagulant therapy for NSTEACS and Mural thrombus following myocardial infarction)
- patients with atrial fibrillation who are to undergo electrical cardioversion or pharmacological cardioversion
- venous thromboembolism prophylaxis
- venous thromboembolism treatment (see Unfractionated heparin for VTE treatment and Thrombolytic therapy and interventional procedures for pulmonary embolism)
- periprocedural bridging therapy of warfarin
- anticoagulation during diagnostic and therapeutic cardiovascular procedures.
