Other drugs to prevent angina
If a combination of 2 of the recommended antianginal drugs (beta blockers, calcium channel blockers or long-acting nitrates) does not provide sufficient symptomatic relief, other drugs can be added. Consider seeking specialist advice.
Nicorandil is an option for patients with refractory angina despite optimal standard therapy. Add to beta-blocker, diltiazem or verapamil therapy:
nicorandil 5 mg orally, twice daily, increasing after 1 week to 10 mg twice daily. If required, increase to 20 mg twice daily. nicorandil nicorandil nicorandil
Perhexiline may be considered for patients who continue to experience angina despite optimal medical management and revascularisation, or if revascularisation is contraindicated (including patient reluctance to undergo an invasive procedure). Seek specialist advice.
