Observation and patient disposition after chronic digoxin accumulation

Admit patients with life-threatening complications due to chronic digoxin accumulation.

In all patients, address any precipitating causes of digoxin accumulation. Withhold digoxin therapy until the serum digoxin concentration returns to the therapeutic range, then consider restarting digoxin at a reduced dose. Stop any P-glycoprotein inhibitory drugs that may have contributed to chronic accumulation of digoxin.
