Vancomycin continuous infusion dosing in young infants not initially treated with an intermittent vancomycin infusion
For young infants aged 0 to 90 days not initially treated with intermittent vancomycin infusion, the continuous infusion should be immediately preceded by a loading dose; use:
vancomycin intravenously (loading dose)vancomycin
15 mg/kg (actual body weight) up to 1 g given over 1 hour as a single dose

vancomycin intravenously (initial continuous infusion dose)
Young infants on a vancomycin continuous infusion should have a random plasma concentration measured 24 hours after starting the infusion – see Vancomycin trough plasma concentration monitoring in young infants and children.
