Vancomycin continuous infusion dosing in children not initially treated with an intermittent vancomycin infusion

For children not initially treated with intermittent vancomycin infusion, the continuous infusion should be immediately preceded by a loading dose; use:

vancomycin intravenously (loading dose)vancomycin

15 mg/kg (actual body weight) up to 1 g given over 1 hour as a single dose

FOLLOWED BY

vancomycin intravenously (initial continuous infusion dose)

60 mg/kg/day (actual body weight) up to 4 g, as continuous infusion.
