Vancomycin plasma concentration monitoring in young infants and children treated with a continuous infusion
For vancomycin continuous infusions in young infants aged 0 to 90 days and children, a random plasma concentration should be measured at steady state (ie at least 24 hours after starting therapy or changing the dose). Random plasma concentrations of 17 to 27 mg/L correlate with an AUC24 of 400 to 650 mg.hr/L.
Note: The recommended vancomycin target random plasma concentration range for young infants and children receiving a continuous infusion is 17 to 27 mg/L.
