Frequency of vancomycin plasma concentration measurement in young infants and children

Irrespective of the vancomycin dosing regimen used in young infants aged 0 to 90 days and children, once 2 plasma concentration measurements (taken 24 to 48 hours apart) are in the target range, monitor the vancomycin concentration at least weekly in stable patients. More frequent measurement is required:

during dose optimisation
if there is a significant change in weight (10 to 20%)
in patients with impaired or changing kidney function
