Dosage of rivaroxaban
Rivaroxaban is a direct factor Xa inhibitor that is taken orally. The dose varies according to the indication and the patient’s calculated creatinine clearance (CrCl) (rivaroxaban is significantly excreted via the kidneys). Do not use rivaroxaban for patients with a CrCl less than 15 mL/min. At the time of writing, rivaroxaban has been studied in a limited number of patients with a CrCl of 15 to 29 mL/min; only consider its use in these patients under advice from a specialistAgeno, 2016Coleman, 2019Lin, 2021Prins, 2014Weir, 2020. Do not use rivaroxaban for patients with severe liver impairment.
The dosage of rivaroxaban varies according to the indication. For example:
- venous thromboembolism prophylaxis following hip or knee replacement
- venous thromboembolism treatment
- stroke prevention in patients with atrial fibrillation
- asymptomatic peripheral artery disease and intermittent claudication in combination with aspirin 100 mg once daily
- secondary prevention of atherosclerotic cardiovascular disease in combination with aspirin 100 mg once daily.
