Management plan for a patient with personality disorder

Collaborate with a patient with personality disorder, as well as their family, carers or significant others, to develop a tailored management plan. The management plan should include:

  • the clinicians involved in treatment and emergency support, including when and how to contact the primary clinician. Clearly defined roles within the treatment team prevent confusion or inconsistency in treatment
  • a case summary and risk assessment, including:
    • short-term and long-term treatment goals
    • current treatments, including psychosocial interventions and any pharmacotherapy
    • strategies that have worked previously for particular presentations
  • how to manage risk of self-harm and suicide (see here for assessing and managing suicide risk)
  • a plan for managing an acute crisis (including emergency department and inpatient treatment plans, if applicable)
  • child protection monitoring if the person has a dependent child.

Review the management plan regularly.
