Clinical presentation of cervical myelopathy

The clinical presentation will vary depending on the site of the lesion. Patients with cervical myelopathy may present with deterioration in fine motor skills, unsteadiness and difficulty walking. On physical examination they have long-tract signs, including unsteady gait, loss of proprioception and a positive Romberg sign. Lhermitte phenomenon (paraesthesia occurring with neck flexion) may be present, but is not specific for myelopathy.
