Compression therapy
Compression therapy is recommended to promote ulcer healing and reduce ulcer recurrence. Aim for class III compression (defined in this guideline as 30 to 40 mmHg). If class III compression is not tolerated, class II compression (20 to 29 mmHg) can be used. Patients often need to start with a lower level of compression (eg a 3-layer tubular bandage) and increase to class III compression when tolerated.
Compression class II or III must be prescribed by an appropriately trained healthcare professional to avoid damage to the limb; see Compression therapy. Frequent review of patients is required, especially at initiation.
Standards of compression vary internationally. For simplicity, classes of compression have been defined in this guideline as shown in Definition of compression classes used in this guideline.
