Overview
Before applying compression therapy, a comprehensive assessment is required to ensure an accurate diagnosis and exclude significant arterial disease (see Assessing patients with an ulcer or wound and Overview of leg ulcers). Undertake clinical assessment of the patient and an ankle brachial pressure index (ABPI).
If ABPI measurement is not available, compression may be used—with close monitoring—if the patient has palpable pedal pulses (dorsalis pedis and posterior tibial), no femoral artery bruit, intact peripheral sensation, adequate cognition to report a complication, and no clinical features that suggest peripheral arterial disease.
