Approach to treatment of primary dysmenorrhoea

Treatment aims to inhibit prostaglandin production, using a hormonal contraceptive or a nonsteroidal anti-inflammatory (NSAID). Hormonal and anti-inflammatory treatments may be combined. If symptoms do not respond within 3 months, arrange an ultrasound and refer for specialist assessment.

Refer earlier if secondary dysmenorrhoea is suspected; early management of underlying conditions, particularly endometriosis, is important for quality of life and fertility preservation.
