Universal prophylaxis

For universal CMV prophylaxis, use:

valganciclovir 900 mg orally, daily. For dosage adjustment in adults with kidney impairment, see valganciclovir prophylaxis dosage adjustment. For allogeneic HSCT recipients with active graft-versus-host disease (GVHD) of the gut, patients with severe diarrhoea, or those unable to tolerate oral therapy, use:

ganciclovir 5 mg/kg intravenously, daily. For dosage adjustment in adults with kidney impairment, see ganciclovir prophylaxis dosage adjustment. Switch to oral valganciclovir (see dosage above) when possible; for duration of prophylaxis, see Assessing the need for antimicrobial prophylaxis in immunocompromised adults without HIV infection. cytomegalovirus, prophylaxis (immunocompromised adult without HIV) ganciclovir    

Some transplant centres use a reduced dosage of ganciclovir (eg given 3 or 5 times weekly).
